It does not contain opinions, interpretations, or conclusions. Every word may be read by a lawyer, a client, or a court. Write accordingly.

Describe each person involved. Use observable physical descriptions β not interpretations or assumptions about intent. If identity is unknown, describe physical appearance, clothing, and behavior only.
SECTION 5 β Incident Narrative
Write in chronological order. Use specific times. State observable facts β not assumptions or interpretations. Write in first person for your own actions, third person for others. Do not editorialize. If you do not know something, write "unknown at time of report" β do not speculate.
SECTION 6 β Actions Taken
List every action taken by this agent. Be specific β "repositioned principal" is not sufficient. State where, how, and why. Check the LE/EMS column if that action involved law enforcement or emergency services.
